NEXT 15 DAYS PSYCHAITRY (27/02/2023 TO 13/03/2023)

NEXT 15 DAYS PERIPHERALS (14/03/2023 TO 29/03/2023)

LAST 15 DAYS UNIT DUTIES (29/03/2023 TO 13/04/2024)

UNIT DUTY : 

I have been posted intially into unit 


Op duties:

Attended my all the 4 op duties and Audit duties

I took history of many  patients and examined them clinically.

Learnt basic management for common conditions like Hypertension,Diabetes mellitus,chest pain , migrane .

And I made PaJR groups and blogs for admitted patients.

-c/o Abdominal distension since 4months

- SOB On  and off  since 4 months


Learning points:

Ascitic tap 

Learnt about SAAG 

Learnt the causes of Low SAAG with high protein and low protein, High SAAG with high and Low protein.

About EEG

Syncope related to arrhythmias

A 55 year old male farmer by occupation came to casualty with cheif complaints of loose stools since 2 days

1 episode of vomiting

Low grade fever

Learning points:

Complications of shock

Management of shock

45 yr old male patient came with c/c of  

-increased  involuntary movements (tremors) ,since 1 day.

-increased drowsiness since 1 day.

Learning points:

Hepatic encephalopathy

Portal hypertension complications

Oesophagial varices

C/o burning sensation in stomach after eating spicy food since 2 months

Cough since 1day


Learning points:

1.Pathophysiology of gastritis 

2.Causes of AKI

3.learned removal of obstructed Foleys catheters

4.management of AKI

5.causes and management of gastritis 



PSYCHIATRY DUTY: 

I have been posted in psychiatry from 29/4/23 to 13/5/23

Saw cases in OPD  and learned how to take basic and detailed history 

I have seen cases of 

1. Generalised Anxiety Disorder 

2. Depression 

3.schizophrenia

4. Post partum Depression 

5. OCD

6.Panic attacks

7.Alcohol Dependence syndrome.


Went to  DAc with pgs for rounds  and saw the process of  treatment and counseling of psychiatry patients.


PERIPHERALS :

I have been posted in icu,amc and Nephro and ward from 14/5/23 to 28/5/23

ICU Duty:

Monitored ICU and AMC patients hourly

Secured cannulas and drawn venous samples 

Took ABG samples and learnt it's interpretation and it's correction.

Inserted Foleys catheters and Condom Catheters

Inserted Ryles tube and given feeding 

Learned about ventilator settings and various modes in ventilator

Assistant in intubation.

Done Cardiopulmonary Resuscitation

Done Dressing for bed sores patient and it's grading and saw all 4 grades .

Thought my junior interns in Foleys insertion ,Ryles tube insertion.



WARD DUTY:

Helped my co interns by updating each and every patient SOAP notes.

Monitoring of ward patients

Drawn blood samples for ward patients.



NEPHRO DUTY:

Monitored the patients undergoing Dialysis.

Learnt about dialysis machine and it's procedure 

Learnt about AV fistula.

Assistant in central line.

And Removed central lines .

Learnt the management of hypoglycemia and hypertension in pts undergoing Dialysis
